Gnathology

THE TREATMENT OF TEMPOROMANDIBULAR JOINT DISORDERS

Gnathology is a branch of dentistry that deals with the diagnosis and treatment of Temporo-Mandibular Joint(TMJ) disorders , also referred to as Temporo Mandibular Disorders(TMD). In TMDs, joint problems affect all structures of the stomatognathic system, with particular involvement of the masticatory muscles and teeth. They can cause severe pain symptoms often accompanied by headache (common headache), also impairing chewing, phonetics, swallowing and even breathing. Causes can be various: stress, dental or skeletal malocclusions, anatomical malformations.

Operating Procedures

TMDs include a number of parafunctions that can lead to pathological conditions such as bruxism, joint clicking , and joint locking. Based on the diagnosis, the most appropriate treatment plan for the patient's clinical condition will be indicated. Through the use of mobile devices called Bite or fixed devices such asbraces, we can improve or resolve the patient's symptomatology.

Bruxism or grinding

Involuntary and improper activation of the chewing muscles
can lead to wear and tear of the teeth, which, by creeping between them repeatedly, will alter
the balance and stability of the mouth. This parafunction can occur in both daytime and
nighttime hours.

Articular Click

Joint click is determined by an incoordination of the joint disc (or meniscus) that is normally located between the mandibular bone head (condyle) and the glenoid cavity of the temporal bone. Under pathological conditions, the joint disc
fails to ensure proper sliding function of the two bony heads resulting in the
typical joint click.

Joint Blockage

A protracted joint problem can result in
irreversible alteration of the TMJ and cause joint blockage, in which the patient will not
be able to perform any kind of movement.

Frequently asked questions

The patient who involuntarily grinds his teeth at night often wakes up with a headache or with muscle tension affecting the cervical area or chewing muscles. In addition, repeated trauma will lead to wear and tear of the teeth, which will be ''shorter'' and/or have irregular margins than normal.

Noises frequently reported by patients with TMD can be traced to condylar-meniscal incoordination, which determines the typical jaw jerk motion and does not allow proper functioning of the temporomandibular joint. Joint noises may often be accompanied by pain near the ear or cheeks.

Incorrect occlusion can result in incorrect positioning of the mandible and upper jaw. Certain problems can be effectively intercepted in the growth phase, a period during which bone structures are forming, while other therapeutic procedures aimed at alleviating or resolving symptoms can be implemented in adulthood.

I had an apicoectomy performed here and I cannot speak highly enough about the entire experience, especially the work done by Dr. Tornabene. The professionalism, precision, and care from the doctor and staff were outstanding from the first visit through follow-up. The staff were incredibly attentive and supportive throughout the entire process. I am originally from New York and spend part of the year living abroad, so finding medical and dental care you truly trust is extremely important to me. I felt completely safe and confident here from day one. I found this clinic after doing extensive research, including using AI tools to compare options, and I am very grateful I did. The level of skill and organization here is exceptional. This was not a simple case, and the team handled it with incredible skill and attention to detail. The surgical result and healing process exceeded my expectations. I would absolutely recommend this clinic and Dr. Tornabene to anyone needing advanced endodontic or surgical dental care. Thank you again for the exceptional work and care.

Today, I had a visit to Dentifree office because of a broken tooth. Something like this happening when you are a few thousand miles away from home and away from your dentist is not fun. So, I was happy to find a friendly place. Dr. Tornabene, Chiara, and Dentifree staff were great in taking care of my tooth. They acted professionally and efficiently, and were respectful. I surely recommend them for dental work.

We arrived in Palermo and my daughter needed work on her braces. We found the staff here to be kind and helpful and the doctor to be very adept. We don't speak Italian and yet we were able to communicate because they were willing to work with us. I highly recommend them!
